     - NTSA Academy Player Registration Form.  The upper right hand corner of this form is to be filled out by your son’s home association (including the box that contains the player registration #). They will need to sign it there as well – no exceptions to that as PAL and any tournament we attend will require this information. You will also need this form notarized.

     - Medical Release Form.  This also requires a notary signature/stamp.
